Game Difficulty window

Game Difficulty can be accessed when starting a new save file by clicking on the Difficulty icon on the lower-left of the New Game screen. There are four different presets available, as well as the option to set up Custom difficulty options.

The following Difficulty settings can be chosen for any type of save, and can be modified in-game as well from the pause menu at the Kerbal Space Center:

The following difficulty settings are exclusive to Career mode saves (and Science saves, for science-specific options), and cannot be changed once a save has been started:
